COVID-19 Team Update: Vaccination Documentation is due Monday, Nov.8

Thank you to the thousands of staff who have already been vaccinated and reported to Occupational Health Services (OHS)! The deadline to report vaccination is rapidly approaching – if you have not already, please send in your documentation by next MONDAY, NOVEMBER 8. See below for further instructions on how to submit your proof of vaccination.

Your decision to become vaccinated protects the health of our entire community and ensures a safe and healthy workplace for all of us. For those of you who are not yet vaccinated, there is still time to protect yourself and those around you! Next Monday, November 8th is the last date you can receive the one-dose Johnson & Johnson COVID-19 vaccine to be fully immunized by November 22. Find a location to get vaccinated as soon as possible.

If you will only be partially vaccinated by November 22 – for example, you have started your Pfizer or Moderna vaccination doses, but have not received your second shot – please send documentation of your first dose to OHS by November 8.

If you are still struggling with the decision to get vaccinated, please consult trusted sources of information. Speak with your doctor about vaccination, review these videos from our Under Secretaries about why they got vaccinated, and visit Vaccines&US, a collaborative Smithsonian website that focuses on the topics surrounding COVID-19 vaccination.

If you are local to the National Capital Region, the DC Department of Health will be hosting a vaccination clinic at the National Zoo on Friday, November 5 from 9:00 a.m. to 2:00 p.m. All three vaccination types will be given as well as boosters. Remember, if you are vaccinated through the National Zoo clinic, you must still report your vaccination status to OHS. Because the vaccine clinic is a DC Department of Health event and not Smithsonian organized, OHS will not receive your records automatically.

To submit your documentation, do one of the following as soon as possible but no later than Monday, November 8:

  • Email an image of your documentation to Occupational Health Services (OHS) at OHS-MedicalReview@si.edu. Please include the name of your unit in the subject line. Or,
  • Fax your documentation to OHS at 202-312-1914, or
  • Bring your document into an OHS clinic for OHS staff to digitize and return the hard copy to you.

Acceptable proof of vaccination documents include:

  • Your CDC COVID-19 Vaccination Record Card
  • A record of your immunization from a health care provider or pharmacy
  • A medical record documenting your vaccination
  • The immunization record from a public health or state immunization information system
  • A copy of any other official documentation containing required information that state:
  • Type of vaccine administered
  • Date(s) of administration and the name of the health care professional(s) or clinic site(s) administering the vaccine(s)

By submitting proof of vaccination, you certify that the documentation you are submitting is true and correct. If you have already submitted a picture of your CDC card to Occupational Health Services, you do NOT need to resubmit. Documentation collected by OHS is maintained confidentially
